The service
FL Provider Forms gives you access to official Florida APD templates, tools to fill them with your facility details, and related documents. The official templates are published by the state and are free. Paid plans cover the filling, locking and packaging tools, not the templates themselves.
Not legal or compliance advice
Nothing on this site is legal advice or a substitute for the rules, handbooks and advisories that govern your license. You are responsible for confirming that a form is the current version and that a completed document meets the requirements that apply to you. Policy templates are starting drafts to be reviewed and adopted by you.
Your account
Keep your sign in details private and enter accurate facility information. Facility locked documents carry the license number you provide. You may not use the service to produce documents for a facility you are not authorized to represent.
Plans and billing
Subscriptions bill monthly through our payment provider and can be cancelled at any time from the billing page. Download limits reset on the first of each month.
Independence
This service is operated by codeAmani Labs LLC and is not affiliated with or endorsed by APD, AHCA or DCF. Agency names and form numbers are used to identify the official documents.
Liability
The service is provided as is. To the extent permitted by Florida law, codeAmani Labs LLC is not liable for losses arising from use of the service, including survey findings, licensing actions or billing outcomes.
