What this site does
FL Provider Forms, operated by codeAmani Labs LLC, compiles official Florida APD templates and lets you fill them with your facility details. It is an independent service and is not affiliated with or endorsed by the Agency for Persons with Disabilities, the Agency for Health Care Administration, or the Department of Children and Families.
What we collect
Your account details (name, email) through our sign in provider, and your facility profile: facility name, license number, address, phone, administrator name and, if you enter it, Medicaid provider number. We record which form you generated and when, so plan limits can be applied. We store billing records through our payment provider.
What we never store
Anything you type into a form about a resident, client or employee is used only to produce your PDF and is not saved. The finished PDF is streamed to your browser and is not written to our storage. This site does not hold completed forms, medication records, incident reports or any other resident health information.
Who we share with
Service providers that run the site: hosting, database, file storage for the official blank templates, sign in, payments and email receipts. They process data on our behalf and may not use it for their own purposes. We do not sell personal information.
Your choices
You can update or delete your facility profile from the dashboard, and ask us to close your account and remove its records by writing to the contact address below.
